On Thursday, August 16, 2018 we will have a special walk-in clinic for Back-to-School shots from 1:00-7:00PM. This will be the last chance for students to get vaccinated before school starts on Monday 8/20, so please help us spread the word!
All patients MUST bring:
- Current shot records* (check with the last doctor’s office where they got shots or school)
- Insurance cards (uninsured and underinsured patients under 19 years old are covered by the West Virginia Vaccines for Children Program)
- Written documentation of school immunization requirements (if not attending a K-12 WV public school)
Patients under 18 years old must be accompanied by their parent or guardian, or another responsible adult with a permission note from the parent or guardian.
